Thanks for requesting a mailed petition form.
After you receive the form and fill it out, please submit it back to us digitally no later than July 28th, or if you’re mailing it back, send it no later than July 24th.
Remember:
- Use a black or blue pen.
- Use the address where you are registered to vote. Check what address here.
- Have a registered Colorado voter witness and sign the form as well. If you don't have someone who can witness, email us at sign@safeandhealthyco.org with the subject "Witness Needed" and we'll schedule a virtual witness session.
- Take a high quality photo of the signed form and upload it, or send it via email to sign@safeandhealthyco.org. You can also mail it to us at Safe & Healthy CO, PO Box #3467, Boulder, CO 80307.
